Proof of Vaccination

Proof Of Vaccination is Required

Updated October 21, 2021

The Province of Ontario requires people to be fully vaccinated and provide proof of their vaccination status to access certain businesses and settings.

In accordance with this requirement, all University Settlement patrons (Members, Clients and Program Users) entering US facilities for sports, fitness, dance and music instruction where a mask may not be worn (e.g. woodwind and voice lessons) and/or recreational activities, including swimming, will need to be fully vaccinated (two doses plus 14 days) and provide proof of vaccination along with photo ID.

Patrons will be required to present their proof of vaccination at the Front Desk for visual inspection along with a piece of government-issued photo ID, e.g., driver’s license, health card, passport, Canadian permanent resident card, etc.

Acceptable proof of vaccination includes Government of Ontario issued vaccine certificate QR code and PDF/paper Receipt of Vaccination. Photocopies of government-issued photo ID and uploads of government-issued photo ID on a phone or other device will now be accepted.

Accommodations will be made to allow individuals in the following circumstances to enter the facility:

  • Children under 12 years old who are not vaccinated.
  • People receiving essential services, including settlement, language and social services, employment services, non-recreation-based seniors services, music instruction where masks are worn will not be required to provide proof of vaccination.
  • Those who cannot receive the COVID-19 vaccine due to medical exemption and have a doctor’s note.

Note: A negative COVID-19 test, or recent COVID-19 infection does not entitle a person to enter the facility for the purposes of sports, fitness, music, and/or recreational activities without providing proof of vaccination.

Masks will continue to be worn by everyone in all public spaces and physical distancing must be maintained.
